What Is DUTCH Testing?

DUTCH stands for Dried Urine Test for Comprehensive Hormones. Rather than a single blood draw, it uses small urine samples that you collect at home on filter paper, typically a few times across a day (and sometimes first thing in the morning and at bedtime). Those dried samples are remarkably stable, easy to mail to the lab, and — for many people — far more convenient than sitting in a lab waiting room or scheduling multiple blood draws.


But the real magic isn’t the collection method. It’s the breadth of what the test can reveal. A DUTCH panel looks at your sex hormones and their metabolites — the downstream compounds your body creates as it processes and clears those hormones. This gives us a window into your unique biochemistry that a standard blood test simply can’t provide.


Beyond a Single Number: The Power of Metabolites

Think of your hormones like a river system. A traditional blood test can tell you how much water is flowing at one particular spot, at one particular moment. That’s useful — but it doesn’t tell you where the water is going, which channels it’s flowing down, or whether some of those channels lead somewhere you’d rather they didn’t.


DUTCH testing maps those channels. Take estrogen as an example. Your body doesn’t just make estrogen and then get rid of it in a single step. It breaks estrogen down along several different pathways. One of these (the 2-hydroxy pathway) is generally considered more favorable, while others (the 4-hydroxy pathway in particular) can produce metabolites that the body ideally wants to neutralize efficiently. DUTCH testing shows how your estrogen is being metabolized and how well your detoxification and methylation processes are keeping up.


This kind of insight is especially valuable for anyone considering or already using hormone therapy. Knowing your metabolic patterns helps your provider tailor a plan that supports healthy hormone processing, not just healthy hormone levels.


A Full Picture of Your Cortisol Rhythm

Sex hormones are only part of the equation. DUTCH testing also gives a detailed look at cortisol, your primary stress hormone, and it does so in a way blood testing struggles to match.


Cortisol is supposed to follow a daily rhythm: higher in the morning to help you wake up and feel alert, then gradually tapering off through the day so you can wind down and sleep at night. When that rhythm gets disrupted — too flat, too high at night, or too low in the morning — it can show up as fatigue, brain fog, trouble sleeping, anxiety, stubborn weight, or that “wired but tired” feeling so many people describe.


Because you collect samples across the day, DUTCH testing captures your cortisol curve rather than a single snapshot. It also distinguishes between your free cortisol (the active portion) and your total metabolized cortisol (a measure of your overall production). Together, these paint a much richer picture of how your stress-response system is really functioning — information that’s hard to get any other way without repeated blood draws.

For many of our patients, the most powerful approach combines information from both, along with a careful review of symptoms and health history.
